DDA Infectious Disease Reporting Form

The Delaware Department of Agriculture has developed a new infectious disease reporting form for use by veterinarians. To assist DDA in having a better understanding of the current situation surrounding infectious diseases in domestic animals in Delaware and the steps we may need to take to stop them from spreading, please submit this form to the Office of the State Veterinarian at DEanimalhealth@delaware.gov.

If there is a sudden increase in cases in your practice of an infectious disease, please report it immediately so we can address the situation with other practices and the public.
